Part 7 of the AI Governance Series

“‘We meant to’ doesn’t pass audits. Evidence does. Logs. Approvals. Overrides. Reviews. That’s the bar now.”

Auditors don’t care what you meant to do. They care what you can prove you did. I’ve watched companies with excellent intentions get destroyed because they couldn’t produce logs. “We have a great culture” doesn’t pass an audit.

You can have the best policies, the most sophisticated framework, and complete executive buy-in. If you can’t prove it’s working, auditors will assume it isn’t.

This is where AI governance gets real.

What Auditors Actually Ask

Forget what you think auditors care about. Here’s what they actually ask when examining AI governance:

  1. “Show me your inventory.” What AI systems are in use? Where did this list come from? When was it last updated?
  2. “Show me the approval.” Who approved this AI use case? What was the approval process? Where’s the documentation?
  3. “Show me the review.” How are AI outputs being validated? Who’s doing the validation? How often?
  4. “Show me the exceptions.” When people deviate from policy, what happens? Where’s the exception log?
  5. “Show me what happens when it goes wrong.” Incident response procedures? Past incidents? How were they handled?

Notice the pattern? “Show me.” Not “tell me.” Not “explain your policy.” Show. Me.

The Evidence Hierarchy

Not all evidence is created equal. Auditors weight evidence based on reliability:

Highest: System-Generated Evidence

Automated logs, timestamps, and records that can’t be easily manipulated. This is gold—unambiguous proof that something happened.

High: Third-Party Documentation

Attestations from external parties, audit reports, vendor certifications. Independent verification increases reliability.

Medium: Internal Documentation

Meeting minutes, approval emails, signed acknowledgments. Self-generated but verifiable with timestamps and attribution.

Low: Self-Attestation

“We do this” without any supporting documentation. Better than nothing, but barely.

The goal is to push as much evidence as possible into the “system-generated” tier. Automation isn’t just efficient—it’s more credible.

AI-Specific Evidence Requirements

AI governance has unique evidence needs beyond traditional IT governance:

  • Discovery Evidence: How you know what AI is being used
  • Classification Evidence: How risk levels were determined
  • Approval Evidence: Who approved each use case and when
  • Training Evidence: Who completed AI awareness training
  • Monitoring Evidence: Ongoing logs of AI usage patterns
  • Review Evidence: Records of human oversight of AI outputs
  • Exception Evidence: Documentation of policy deviations
  • Incident Evidence: Records of AI-related incidents and responses

Building the Evidence Trail

Evidence collection needs to be baked into your governance processes from the start. Here’s how:

1. Automate Discovery Logging

Every time your discovery tools detect AI usage, log it: timestamp, user, tool, data classification. This creates the foundation for everything else.

2. Workflow-Based Approvals

Don’t approve AI use cases via email. Use a workflow system that captures: who requested, what was requested, who approved, when, and under what conditions.

3. Attestation Automation

Policy acknowledgments and training completions should be captured with timestamps and stored centrally. No spreadsheets—use a system of record.

4. Monitoring Dashboards

Your monitoring tools should produce evidence as a byproduct: usage trends, policy violations, exception requests. Screenshot these regularly for audit packages.

5. Incident Tickets

Every AI-related incident—even minor ones—should generate a ticket with documented investigation and resolution.

The Evidence Lifecycle

Evidence isn’t just collected. It needs to be managed:

  • Collection: Automated where possible, documented where manual
  • Storage: Centralized, tamper-evident, backed up
  • Retention: Keep evidence based on regulatory requirements (typically 3-7 years)
  • Retrieval: Indexed and searchable for audit response
  • Presentation: Formatted for auditor consumption

Common Evidence Gaps

Where do organizations typically fail on AI governance evidence?

Gap 1: No Discovery Documentation

“We know what AI tools are being used” but no documented inventory, no discovery methodology, no update schedule. Auditors see this as: “They don’t actually know.”

Gap 2: Informal Approvals

“The manager said it was OK” via Slack or verbal approval. No documentation = no approval in audit terms.

Gap 3: No Review Records

Policy says “human review required” but no evidence that reviews actually happen. Who reviewed? What did they check? When?

Gap 4: Missing Exception Trail

Exceptions are granted but not documented. No evidence of who approved, why, or what conditions were attached.

Continuous vs. Point-in-Time Evidence

Annual audits don’t mean annual evidence. Auditors want to see continuous operation:

  • Point-in-time: “Here’s our policy as of today” (necessary but insufficient)
  • Continuous: “Here’s our monitoring data for the past 12 months” (what auditors really want)

Continuous evidence demonstrates that controls operate consistently, not just during audit season.

Preparing for the AI Audit

When an auditor comes asking about AI governance, you should be able to produce within hours:

  • Current AI tool inventory with risk classifications
  • AI governance policies with approval dates and review history
  • Training completion records for the audit period
  • Approval workflow records for all AI use cases
  • Monitoring reports showing policy compliance over time
  • Exception log with approvals and justifications
  • Incident records with investigation and resolution documentation
  • Evidence of regular governance reviews and updates

If producing this package takes weeks, you have an evidence problem.

The Risk Scoring Reality

Many organizations create AI risk scores. Few can defend them.

Auditors will ask:

  • What methodology did you use?
  • What factors were considered?
  • Who assigned the scores?
  • How often are they reviewed?
  • Show me an example of how a score changed when risk changed.

If you can’t explain how you got the score, you’re not scoring risk. You’re generating comfort theater. Auditors will shred you.
